PARENT/LEGAL GUARDIAN CONSENT FORM 
 

Contestants must be 18 years of age on the date of submitting a completed Nurse Heroes art contest registration form.  Contestants under the age of 18 must have a parent or eligible legal guardian complete and submit a completed contest enrollment consent form.  The completed form must be uploaded at the time of registering for the Nurse Heroes art contest.
